By clicking "Pay," "Complete Purchase," "Authorize Payment," or by otherwise submitting payment through the Better by MTA platform ("Platform"), you acknowledge that you have read, understood, and agree to be legally bound by these Payment Terms and Conditions.
These Terms constitute a legally binding agreement between you ("Customer," "Patient," "Employer," "Executive," "Purchaser," or "User") and Healthcare Revolution, LLC ("Healthcare Revolution," "HREV," "we," "our," or "us").
If you do not agree to these Terms, do not proceed with payment.
Better by MTA is a technology platform operated by Healthcare Revolution, LLC.
The Platform facilitates connections between patients, employers, executives, facilitators, insurers, wellness organizations, healthcare providers, longevity clinics, hospitals, physicians, diagnostic centers, and other healthcare-related organizations worldwide.
Healthcare Revolution does not own, manage, supervise, or control the independent healthcare providers listed on the Platform unless expressly stated.
Healthcare Revolution is not the provider of medical care.
Healthcare Revolution is not responsible for diagnosing medical conditions, recommending treatments, practicing medicine, or guaranteeing medical outcomes.
Payments made through the Platform may include, without limitation:
Healthcare Revolution may collect payments on behalf of participating providers or may collect payments for Platform services, administrative services, technology services, or other authorized purposes.
By submitting payment, you authorize Healthcare Revolution and its authorized payment processors, including Stripe and its banking partners, to charge your selected payment method for:
You represent that you are authorized to use the payment method submitted.
Payments are securely processed through Stripe and other authorized payment partners.
Healthcare Revolution does not store full payment card information.
Payment processing is subject to the terms, policies, fraud prevention procedures, and security requirements of Stripe and applicable financial institutions.
Healthcare Revolution reserves the right to delay, reject, cancel, or refund transactions where fraud, unauthorized activity, regulatory requirements, sanctions, payment disputes, or technical issues are suspected.
Payments made through the Platform do not create a physician-patient relationship with Healthcare Revolution.
Healthcare Revolution does not provide:
All medical decisions remain solely between the customer and the selected healthcare provider.
Certain payments may represent deposits rather than full payment.
Deposits may reserve:
Deposit requirements vary by provider.
Unless otherwise stated in writing, deposits are non-refundable.
Except where required by applicable law or expressly agreed to in writing by Healthcare Revolution or the applicable provider, all payments made through the Platform are final.
Payments may include substantial administrative work, provider scheduling, payment processing expenses, reserved appointment capacity, and technology platform services that begin immediately upon payment.
Accordingly:
Where payment is made on behalf of an independent healthcare provider, any refund relating to medical services shall be determined solely by the provider in accordance with its own cancellation policies.
Healthcare Revolution has no obligation to issue refunds for services performed or scheduled by independent providers.
Healthcare Revolution may, in its sole discretion, issue partial refunds, credits, or payment adjustments.
Any such action shall not establish a precedent or create an obligation to issue future refunds.
Cancelling an appointment does not automatically entitle you to a refund.
Provider cancellation policies vary.
Cancellation requests must be submitted in writing.
Failure to attend a scheduled appointment ("no-show") may result in forfeiture of all payments made.
You agree not to initiate an unwarranted chargeback, payment dispute, or payment reversal.
Before filing a dispute with your financial institution, you agree to contact Healthcare Revolution so we may attempt to resolve the matter.
Improper chargebacks may result in:
Healthcare Revolution reserves the right to provide payment records, signed agreements, electronic acknowledgments, communications, invoices, and other evidence to financial institutions in response to chargeback claims.
Where financing or Buy Now Pay Later services are offered, financing is provided by independent financial institutions.
Healthcare Revolution does not:
Financing agreements are solely between the customer and the financing provider.
Payments may be accepted in multiple currencies.
Exchange rates may fluctuate.
Healthcare Revolution is not responsible for:
Customers remain responsible for any taxes, duties, governmental fees, or assessments applicable to their transactions unless expressly included in the quoted amount.
Healthcare Revolution is not responsible for:
Healthcare providers remain solely responsible for the healthcare services they provide.
Healthcare Revolution makes no guarantee regarding:
Healthcare Revolution may modify, suspend, discontinue, or update the Platform at any time without prior notice.
We do not guarantee uninterrupted access.
Healthcare Revolution reserves the right to:
You consent to receive communications electronically regarding your payment, appointments, receipts, invoices, account activity, and customer support.
Electronic records satisfy any legal requirement that communications be in writing.
To the fullest extent permitted by law, Healthcare Revolution, its affiliates, officers, directors, employees, contractors, licensors, technology providers, payment processors, and partners shall not be liable for any indirect, incidental, consequential, special, exemplary, or punitive damages, including loss of profits, loss of opportunity, travel expenses, medical expenses, business interruption, or emotional distress arising out of or relating to your use of the Platform or any services purchased through it.
Healthcare Revolution's total aggregate liability arising from any claim relating to a payment shall not exceed the amount actually paid by the customer through the Platform for the specific transaction giving rise to the claim.
You agree to defend, indemnify, and hold harmless Healthcare Revolution, its affiliates, officers, employees, contractors, licensors, and partners from any claims, damages, liabilities, costs, or expenses (including reasonable attorneys' fees) arising out of:
Healthcare Revolution shall not be liable for delays or failures caused by events beyond its reasonable control, including natural disasters, pandemics, governmental actions, war, labor disputes, cyberattacks, internet failures, or interruptions in payment processing services.
These Terms shall be governed by and construed in accordance with the laws of the State of Florida, without regard to conflict of law principles.
Any legal proceeding arising under these Terms shall be brought exclusively in the state or federal courts located in Palm Beach County, Florida, and you consent to the jurisdiction of those courts.
If any provision of these Terms is determined to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
These Terms constitute the entire agreement regarding payment transactions conducted through the Platform and supersede all prior discussions, representations, or agreements relating to such payments.
Healthcare Revolution may update these Terms at any time by posting a revised version on its website or Platform. Updated Terms become effective upon posting unless otherwise stated.
